140####### Position Summary Renders direct and indirect care to assigned patient populations.
Assess patients and evaluates bio-psycho-social needs, coordination of in-home services, and referrals to community resources.
Collaborates with other members of the social service team to assure the appropriate provision of social services and other health care teams utilizing a holistic approach to patient care.
Responsible for the social work services and treatment of all ages including infants, child, adolescent, young adult, adult, and geriatric.
Service may include family members, and from any other concerned party.
Qualifications Minimum 1.
A Masters Degree in Social Work, or other related field.
2.
Must possess and maintain a current Basic Life Support (BLS) certificate.
Desired 1.
Eligible for licensure as a Licensed Clinical Social Worker or a Marriage Family Child Counselor through the California Board of Behavioral Science Examiners 2.
One-year prior experience in an acute care setting or skilled nursing facility.
3.
Two years of full time medical social work in a public or private institution or private services agency.
